The Patient Portal is an online tool that offers patients a secure environment to perform functions like requesting refills on medications, looking at lab results and communicating with their providers in a confidential and secure manner. Although patients can send messages to the provider/staff, the provider/staff cannot send messages back. Our office policy is all messages received through patient portal including refill request will be addressed in 2-3 business days.

Our office mobilizes Health Companion. Health Companion is a place to store, manage, and communicate your health information with your doctor, insurance company, employer and family.

 

 

To get started, follow these easy steps:

  1. Go to https://www.healthcompanion.com/falcon 

  2. Enter your PIN (Given By Our Office), first name, last name, and date of birth                                                                                   Note: The PIN is valid for 30 days following the date of this letter. After 30 days you will need to ask your physician to generate a new PIN for you to use. ​

  3. Click the Search button to continue the process.

  4. Click the Terms and Conditions agreement checkbox that appears.

 

Once you have marked the Terms and Conditions checkbox, you will be prompted with the following questions:

 

Are you the individual identified above? Please mark "Yes" if that statement is true.

Are you already registered as a user in Health Companion? Please select "No"

 

Next, create your username, password and select three security questions and answers. After you enter in this information, please click Sign Up. Your registration is then complete.

 

To Login to your new patient portal, enter the new username and password you just created.

You will then be directed to your Health Companion account and will have access to your health information.

In your portal, you will find:

  • Your health information including labs, health history, family history, etc.

  • Tools for managing your medical bills, insurance claims, and health accounts (FSA, HSA, etc.)

  • Personalized preventive health information and reminders and secure communication with your health network. Please note that our office cannot message you back at this time. We can contact you via phone or answer questions in your next appointment.
