Office Policy

Our doctor will care for all patients to the best of their abilities.

Before receiving any test results from office staff you will be required to provide two patient identifiers (Name, Date of birth, or last four digits of Social Security)

 

Occasionally, it may be possible our doctor may be called away from the office in a hospital emergency. In that event, our office will try and accommodate you and try and call you as soon as possible.

Prescriptions/ Refills

We refill prescriptions ONLY prescribed by our doctors. Any non-nephrology/hypertension medications need to be filled with your primary care doctor or specialist that prescribed the medication. 

 

Please allow up to 2 days for prescriptions to be filled. No prescriptions will be filled Friday- Sunday. Any request for refills during those days will be addressed the following Monday.

 

Our office may ask for lab testing before a prescription is written.

New Patients

We do not draw labs in the office therefore you do not have to fast before any appointment unless an office member informs you to do so.

 

 

How to be Prepared for your appointment
  • Please arrive 15 minutes early to your appointment.

  • Bring all medication BOTTLES to every appointment.

  • Bring all insurance cards and notify the office of any changes of address and/or phone numbers
